CLINICAL AND SURGICAL EXPERIENCE


During my otolaryngological training in Bombay, Bangalore, the West of Scotland and the West Midlands I have gained experience in all aspects of Clinical Otolaryngology. At KEM hospital and Municipal ENT hospital, Bombay, I rotated through three clinical firms each with specialised areas of interest and was thereby exposed to otology, neurotology, rhinology, paediatric otolaryngology and head & neck surgery. This experience has been consolidated by a similar arrangement at Stafford, Glasgow, Walsall and Birmingham. As Senior Consultant, Assistant Professor and Head of Unit 2 in the Department of Otolaryngology at St John’s National Academy of Health Sciences, Bangalore, India, the expertise in the above mentioned fields were put into successful practice. Presently, I am in private practice in Dubai, UAE working for the Canadian Specialist Hospital that has a special focus on genetic disorders management. A deafness rehabilitation programme has been set up to include cochlear implantation and bone anchored hearing aid technology.

Otology, Neurotology and Skull Base Surgery:

The ENT training that I received in India and in the West, including UK and Canada, has provided me with a very broad clinical and operative experience in Otology and Neurotology. I have attended dedicated otology lists in which I have performed a wide variety of middle ear and mastoid surgeries under supervision and independently. I have participated in the management of hearing-impaired children and adults. This included the use of bone-anchored hearing aids, middle ear implantable hearing devices and cochlear implants (Stafford, Glasgow, Walsall, Birmingham and Bangalore).  

In Birmingham, Glasgow and Toronto, I have been exposed to the various surgical approaches available for the removal of acoustic neuromas. I have also been exposed to the Middle Cranial Fossa (MCF) approach for CSF leak closure, IAM, CPA and petroclival tumours. As Specialist Registrar subspecialising in Otology and Neurotology (Years 5 and 6) and as Clinical Fellow in Toronto, I gained experience in middle ear surgery including ossicular chain reconstruction, combined approach tympanoplasty (CAT), revision mastoid surgery and inner ear surgeries. I have received substantial training in Bone Anchored Hearing Aids (BAHA), Facial and Audiological Implant Technology (FAITEC) and Cochlear Implantation (CI), both adult and paediatric. The Otology group in Birmingham is one of the few units in the world that has evaluated the benefits of the Vibrant Soundbridge Device.

In Bangalore, I was able to set up a Cochlear Implant programme affiliated to the Cochlear Implant Group of India (CIGI). I was the Founder Secretary of the CIGI (2003-2004). I have been able to start a CI programme in Dubai as well.

Specialist Clinics in Otology and Neurotology:

I have been involved in the organisation and management of the Vertigo and Tinnitus clinics at Bombay, Glasgow, Birmingham and Toronto. I have been involved in the running of the following special clinics - Neurotology, Bone Anchored Hearing Aids, Cochlear Implantation and FAITEC. I am quite conversant with the clinical evaluation of patients attending the above clinics and the battery of tests involved in their assessment and hence management protocols.

Otology and Neurotology Research: Dissertations

The dissertation topic for Master of Surgery (MS-ENT) accepted and published by the Bombay University was on Post-meningitic deafness. This work was carried out in KEM Hospital, Bombay under the tutelage of Prof MV Ingle and Prof MV Kirtane. I have since been involved in clinical research in Otology and Neurotology and have published and presented papers in these fields. The work I have done on 'Outcome evaluation with the BAHA' over three years in Birmingham has culminated in the award of a Ph D at Nijmegen University, Netherlands in 2002 under the promotion of Prof Cor Cremers, Nijmegen and Mr David Proops, Birmingham. In Toronto, I am currently engaged in evaluating investigations and surgeries for vertigo under the tutelage of Dr John Rutka. With my colleagues, I am also involved in evaluating outcomes following stapes replacement surgery, a case series by Dr Jerry Halik.

Rhinology:

I have participated in specialist rhinology clinics and am proficient in the use of the rigid nasoendoscope for diagnosis in this context. Otolaryngology practice in Bombay, the West of Scotland and the West Midlands experienced referral of many patients with nasal trauma and I am now able to assess and manage both minor and severe nasal and facial injuries.  

In Bombay, I was involved in the management of a large series of patients with nasopharyngeal angiofibroma. I have performed and assisted with various surgical procedures including lateral rhinotomy, transnasal-maxillary, transpalatal and midfacial degloving approaches.  

The allergy and rhinology clinics in Bangalore, Glasgow and Birmingham have developed my awareness and interest in the immunological aspects of sinonasal disorders. I am presently conversant with functional endoscopic sinonasal surgery (FESS) in the management of Sinonasal diseases. I have also attended some of the recommended courses in FESS at Bombay, Dundee and Brussels-Cologne, towards this end.

Laryngology and Head and Neck Surgery:

Bombay, the West Midlands and the West of Scotland provided a rich seam of head and neck cancers. My Otolaryngology, Plastic Surgery and General Surgery posts provided extensive opportunities to be involved in the clinical and operative management of head and neck patients. I am familiar with the running of combined head and neck clinics and procedures for second primary screening by virtue of exposure to the same at Stafford, Walsall and at Beatson Oncology Centre at Glasgow’s Western Infirmary.

I am familiar with the use of carbon dioxide and KTP 532 LASER in Otolaryngology, specifically with its use in the treatment of benign laryngeal disease. I am a Nominated LASER User (NLU). I have a sound knowledge of management of benign laryngological diseases having been involved in the 'Voice clinics' at Glasgow and Birmingham.

Paediatric Otorhinolaryngology:

I have experience in the assessment and management of common paediatric disorders and of rare conditions referred to tertiary referral centres such as KEM, Bombay and The Royal Hospital for Sick Children, Glasgow, Diana Princess of Wales Children's Hospital, Birmingham. I have participated in routine outpatient clinics and in special clinics for audiological assessment and the review of high-risk infants.  

I was involved in a prospective study on the benefits of FESS in children with rhinosinusitis, cystic fibrosis and mucociliary disorders at RHSC, Glasgow.

A substantial part of my training in Birmingham has been in Paediatric Otology. The training I have received in the work-up and the management of 'deafness in children' and in the use of BAHA and Cochlear Implants in children is of a high standard.

As senior registrar at the Diana, Princess of Wales Children's Hospital, Birmingham, I have developed a thorough insight into the management of paediatric airway ailments.

General and Plastic Surgery:

Periods spent in General Surgery and allied specialties including Plastic Surgery at KEM Hospital, Bombay have enabled me to put the management of otolaryngological patients in a broadly based clinical context. This includes surgical experience in the form of burr hole preparations, abdominal surgeries that include bowel surgery, hepatobiliary surgery, breast surgeries and major head and neck resections with myocutaneous flap reconstructions.  

Some of the procedures performed and assisted include pectoralis major myocutaneous flap (PMMC), delto-pectoral and forehead flaps, hand and wrist surgery including nerve and tendon repair and k-nailing and splinting of finger injuries, delayed groin flaps for degloving injuries and skin loss, skin grafting techniques for burns, facial nerve grafting, pinnaplasty, rhinoplasty, cleft lip and palate surgery.

OUTPATIENT CLINIC EXPERIENCE

I have independently managed new and follow-up patients. I am proficient with the use of rigid and flexible nasal endoscopes and laryngoscopes and the operating microscope for minor ear procedures.

RESEARCH EXPERIENCE


MS Dissertation: Bombay University (1994)

* Dutt SN, Prabhuzantye S, Ingle MV, Kirtane MV (1994); Postmeningitic deafness in children: Evaluation and localization of the site of pathology by Brainstem Evoked Response Audiometry (BERA).



The dissertation topic was the use of Brainstem Evoked Response Audiometry in establishing patterns of sensorineural deafness in paediatric meningitis. 25 of the 44 cases of meningitis studied in the 6 months to 3 years age group had sensorineural hearing loss. It was also possible to localize the site of pathology by looking at waves I, III and V and interwave intervals.

It was interesting to note that in cases of pyogenic meningitis the deafness was predominantly cochlear and in some cases reversible. In cases of tuberculous meningitis, the deafness was retrocochlear and was irreversible.

* Dutt SN (2002). The Birmingham Bone Anchored Hearing Aid Programme: Some Audiological and Quality of Life Outcomes.



Since it’s inception in 1988, the Birmingham BAHA Programme has implanted more than 350 patients with a Bone Anchored Hearing Aid (BAHA). These include both adults and children. A questionnaire survey was undertaken to evaluate patient satisfaction and quality of life issues. The questionnaires used included

* The Glasgow Benefit Inventory (GBI)
* The Nijmegen group questionnaire for intraindividual comparison of aids
* The Entific Medical Systems questionnaire for evaluation of day-to-day use and service related issues and finally
* The Glasgow Hearing Aid Benefit and Difference Profiles (GHAB/GHADP).

The study showed a high degree of patient satisfaction with the BAHA.

The study cohort also included 15 patients with bilaterally fitted BAHAs. 11 of these patients were subjectively evaluated with patient satisfaction (GBI) and binaural hearing questionnaires (Chung and Stephens questionnaire). The results showed a high degree of satisfaction and improved quality of life with the second BAHA. Objective evaluation using a battery of audiological tests revealed that bilateral fitting of BAHA enabled binaural hearing. Sound localisation, speech perception in noise and stereophonic perception of sound were the features that were better with the two BAHAs compared to unilateral BAHA.

A Knowledge, Attitude and Practice (KAP) study regarding the prescription of bilateral hearing aids amongst ENT practitioners in the UK revealed a poor awareness, indifferent attitude and poor practice predominantly due to financial constraints.

Finally, a surgical technique that addresses and hence limits the wound complications at the placement of the BAHA fixture and abutment has been described.
