Cardiology



Aneurysm Treatment

An aneurysm is an abnormal bulge in the wall of a blood vessel. They can form in any blood vessel, but they occur most commonly in the aorta (aortic aneurysm) which is the main blood vessel leaving the heart. The two types of aortic aneurysm are:
Thoracic aortic aneurysm (part of aorta in the chest)
Abdominal aortic aneurysm

Small aneurysms generally pose no threat. However, one is at increased risk for:
Atherosclerotic plaque (fat and calcium deposits) formation at the site of the aneurysm
A clot (thrombus) may form at the site and dislodge
Increase in the aneurysm size, causing it to press on other organs, causing pain
Aneurysm rupture - because the artery wall thins at this spot, it is fragile and may burst under stress. A sudden rupture of an aortic aneurysm may be life threatening
Treatment options include stenting as well as surgical repair.



Coronary Bypass (CABG)
A Coronary Artery Bypass Graft (CABG) is a form of bypass surgery that creates new routes around narrowed and blocked coronary arteries to improve the supply of blood and oxygen to the heart. CABG is an option for selected groups of patients with significant narrowing and blockages of the heart arteries. The bypass graft for a CABG can be a vein from the leg or an inner chest-wall artery. CABG surgery is one of the most commonly performed major operations. Coronary artery disease develops because of hardening of the arteries (atherosclerosis) that supply blood to the heart muscle. CABG results in an improved blood flow to the heart that can relieve chest pain (angina).

Heart Valve Surgery
Heart valve surgery can be for repair or replacement of a damaged valve in the heart. In heart valve replacement surgeons remove a damaged valve from the heart and substitute a replacement valve. There are three types of replacement valves: animal tissue, mechanical and human valves (from a donor or, rarely, the patient's own pulmonic valve). Most valve replacements or repairs are performed for mitral or aortic valves. Patients are generally very closely monitored in the ICU following surgery but once stabilization is ensured the patient is transferred to a standard medical/surgical unit. As soon as possible, the patient begins walking and exercising to regain strength. After being released from the hospital, the patient is instructed to continue a daily exercise program, maintain a diet low in salt and cholesterol and join a cardiac rehabilitation program. Most patients who have these surgeries return to normal activity after recuperation.

Angioplasty

Balloon angioplasty is a technique used to unblock coronary arteries that have been clogged due to atherosclerosis. During balloon angioplasty, the doctor guides a small catheter into a blood vessel from the groin or arm, and guided with the assistance, and to the blockage. The catheter is tipped with an inflatable balloon. Once the balloon reaches the site of obstruction, it is rapidly inflated, crushing the plaque against the arterial wall and restoring blood flow through the vessel. To help keep the vessel open, the physician might also permanently implant a tiny mesh tube called a stent.

Spinal



Spinal Fusion
Spinal fusion (arthrodesis), also known as spondylosyndesis is a surgical technique used to combine two or more vertebrae. Supplementary bone tissue (either autograft or allograft) is used in conjunction with the body's natural osteoblastic processes. This procedure is used primarily to eliminate the pain caused by abnormal motion of the vertebrae by immobilizing the vertebrae themselves.
Spinal fusion is done most commonly in the lumbar region of the spine, but it is also used to treat cervical and thoracic problems. People rarely have problems with the thoracic spine because there is little normal motion in the thoracic spine. Patients requiring spinal fusion have either neurological deficits or severe pain which has not responded to conservative treatment.

Cosmetic Surgery



Tummy Tuck

Abdominoplasty, known more commonly as a "tummy tuck," is a major surgical procedure to remove excess skin and fat from the middle and lower abdomen and to tighten the muscles of the abdominal wall. In an abdominoplasty, the surgeon will usually make a long incision from hipbone to hipbone just above the pubic area. A second incision is made to free the navel from surrounding tissue. Abdominal muscles are then tightened providing a firmer abdominal wall and narrowing the waist. With partial abdominoplasty, the incision is much shorter and the navel may not be moved, although it may be changed in shape in the process. The procedure can dramatically reduce the appearance of a protruding abdomen.



Blepharoplasty

Blepharoplasty can be both a functional or cosmetic surgical procedure intended to reshape the upper eyelid or lower eyelid by the removal and/or repositioning of excess tissue as well as by reinforcement of surrounding muscles and tendons. When an advanced amount of upper eyelid skin is present, the skin may hang over the eyelashes and cause of loss of peripheral vision. The outer and upper parts of the visual field are most commonly affected and the condition may cause difficulty with activities such as driving or reading. In this circumstance, upper eyelid blepharoplasty is performed to improve peripheral vision. Patients with a less severe amount of excess skin may have a similar procedure performed for cosmetic reasons. Lower eyelid blepharoplasty is almost always done for cosmetic reasons, to improve puffy lower eyelid "bags" and reduce the wrinkling of skin.


Rhinoplasty

Rhinoplasty is a type of plastic surgery that is used to improve the appearance (cosmetic surgery) of a person's nose. Rhinoplasty is also commonly called a "nose job." Rhinoplasty can be performed to meet aesthetic goals or for reconstructive purposes to correct birth defects or breathing problems. It can be combined with other surgical procedures such as chin augmentation to enhance the aesthetic results.


Facial Surgery

Facial surgery involves the application of one or more surgical techniques to enhance the appearance of the different structures of the face. This might include the use of dermal fillers, implants, liposuction or reconstruction to achieve the desired result.


Breast Surgery

Breast Surgery includes:

Breast augmentation (augmentation mammoplasty) is a surgical procedure that inserts implants to enhance the size and shape of a woman's breast. It can be done for purely cosmetic reasons or as reconstruction following breast surgery. Incisions are placed so resulting scars will be as inconspicuous as possible. Working through the incision, the surgeon creates a pocket, and the implants are then centered beneath the nipples. Patients can often return to normal activities within a couple of days, with some restrictions.

Breast reduction is a surgical procedure performed in order to decrease the size of the breasts. Women with very large breasts (macromastia or mammary hyperplasia) seek breast reduction for relief of pain in the back, shoulder, and neck. They may also feel uncomfortable about their breast size and have difficulty finding clothing that will fit properly. Additionally, breast reduction may be needed after reconstructive surgery following the surgical removal of cancerous breast tissue (mastectomy), to make the breasts more symmetric. Men who have enlarged breasts (gynecomastia) may also be candidates for breast reduction. However, excessive alcohol intake, smoking marijuana, or using anabolic steroids may cause gynecomastia, and surgery is not recommended for men who continue to use these products. Breast reduction is not recommended for women whose breasts are not fully developed or who plan to breast feed.

Breast Lift Over the years, factors such as pregnancy, nursing, and the force of gravity can affect the appearance of a woman's breasts. As the skin loses its elasticity, the breasts often lose their shape and firmness and begin to sag. A breast lift, or mastopexy, is a surgical procedure to raise and reshape sagging breasts. Mastopexy can also reduce the size of the areola, the darker skin surrounding the nipple. If your breasts are small or have lost volume — for example, after pregnancy — breast implants inserted in conjunction with mastopexy can increase both their firmness and their size. Breasts of any size can be lifted, but the results may not last as long in heavy breasts.

Dental


Deep Cleaning

Deep cleaning is a cleaning procedure performed by the dental hygienist. It is a non-surgical treatment for gum disease. It is recommended when one needs more work than a regular professional cleaning. The objective of deep cleaning is to remove deposits of bacterial plaque, tartar (calculus), food debris and diseased debris that have accumulated in the infected pocket of the tooth and then to smooth the root in order to eliminate any rough tartar deposits on the root which can provide breeding grounds for bacteria.


Dental Bridge Work

A dental bridge, otherwise known as a fixed partial denture, is a prosthesis used to replace missing teeth and is not removable by the patient. A prosthesis that is removable by the patient is called a removable partial denture.
A dental bridge is fabricated by reducing the teeth on either side of the missing tooth or teeth by a preparation pattern determined by the location of the teeth and by the material from which the bridge is fabricated. In other words the abutment teeth are reduced in size to accommodate the material to be used to restore the size and shape of the original teeth in a correct alignment and contact with the opposing teeth.


Apicoectomy

This procedure may also called Root End Resection or Root End Amputation. It is a surgical procedure to remove the tip or end of the root of a tooth lying in the bone of the jaws, through an opening made in the buccal (cheek side) or palatal side of the tooth. At the same time, tissue from around the apex or end of the root may be removed to control a disease process or to facilitate healing. The procedure is usually associated with Root Canal Treatment, either during the procedure, or more often afterwards, if an earlier root canal treatment has not healed satisfactorily.



Crowns

Crowns are a type of dental restoration which, when cemented into place, fully cap the portion of a tooth which lies at and above the gum line. The difference between a crown and a filling is that a filling is used to fill in or cover just a portion of a tooth. Crowns cover the entire visible aspect of a tooth; a dental crown in effect becomes the tooth's new surface. Crowns can be made out of porcelain (or some other ceramic material), gold (or metal alloys), or a combination of both. Dental crowns are often referred to as "dental caps" or "tooth caps".


Dentures

Dentures are prosthetic teeth constructed to replace missing teeth, which are supported by surrounding soft and hard tissues of the oral cavity. Conventional dentures are removable, however there are many different denture designs, some which rely on bonding or clipping onto teeth or dental implants.



Dental Implants

A dental implant is a specially designed small cylinder screw made of a biologically compatible Titanium.
Implants act as an anchor and can be used for replacing:
a single tooth,
one or both ends of a bridge or
a full denture.

An implant is surgically set into the jawbone. The bonding of bone to the implant usually takes four to six months. During this time the implants cannot be used and the patient wears a temporary bridge for aesthetic purposes. Once the implant has bonded to the bone a small metal post is attached to the implant, after this step the gum must heal a few days prior to the crown being placed.

Immediate load implants, implant in an hour, are also available to suitable candidates. The dentist performing the procedure will assess a candidate’s suitability for this procedure upon office visit.



Root Canal

At the center of a tooth is a hollow area that houses soft tissue, known as pulp. This hollow area contains a relatively large space towards the chewing surface of the tooth called the pulp chamber. This pulp chamber is connected to the tip of the tooth root via thin hollow pipe-like canals—hence, the term "root canal".
Occasionally, a cavity on the outer surface of the tooth may cause this soft tissue to become inflamed or infected. Left untreated, a serious jaw infection can result.
Sometimes a tooth is considered so threatened (because of decay, cracking, etc.) that future infection is considered likely or inevitable, and a pulpectomy, removal of the pulp tissue, is advisable to prevent it. But usually some inflammation and/or infection is already present, within or below the tooth. To cure the infection and save the tooth, it is necessary for the dentist to drill into the pulp chamber, and remove the infected pulp by scraping it out of the root canals. Once that is done, the dentist fills the cavity with an inert material and seals up the opening. This procedure is known as root canal therapy. If enough of the tooth has been damaged, or removed as a result of the treatment, a crown may be required.

Tooth Whitening
A whitening gel is placed in a thin bleaching tray that fits over your teeth. As the active ingredient in the gel, carbamide peroxide, is broken down, oxygen enters the enamel and dentine and bleaches the colored substances. The structure of the tooth is not changed, only the tooth color is made lighter.

Porcelain veneers are thin shells of porcelain that are placed on the front of existing teeth to change the length, width, color and shape of a tooth. They are the greatest tool for enhancing a smile, they are versatile and strong. Veneers are used for a variety of smile enhancements, from complete smile makeovers, repairing a chipped tooth, restoring teeth to their natural beauty. Veneers also resist staining, creating an enduring white, bright, straight smile.

Porcelain Inlays and overlays are used when someone looses more than 50% of the natural crown or the tooth needs strengthening. They are made of porcelain to match the natural color of the remaining portion of the tooth. Porcelain inlays or over lays provides an affordable and cosmetic solution to chipped, stained, or slightly crooked teeth.

General Surgery



Colonoscopy

Colonoscopy is the minimally invasive endoscopic examination of the large colon and the distal part of the small bowel with a CCD camera or a fiber optic camera on a flexible tube passed through the anus. It may provide a visual diagnosis (e.g. ulceration, polyps) and grants the opportunity for biopsy or removal of suspected lesions. Virtual colonoscopy, which uses 2D and 3D imagery reconstructed from computed tomography (CT) scans or from nuclear magnetic resonance (MR) scans, is also possible, as a totally non-invasive medical test, although it is not standard and still under investigation regarding its diagnostic abilities. Furthermore, virtual colonoscopy does not allow for therapeutic maneuvers such as polyp/tumor removal or biopsy nor visualization of lesions smaller than 5 millimeters. If a growth or polyp is detected using CT colonography, a standard colonoscopy would still need to be performed. Colonoscopy can remove polyps smaller than one millimeter. Once polyps are removed, they can be studied with the aid of a microscope to determine if they are precancerous or not.



Laparoscopic Cholecystectomy

Laparoscopic Cholecystectomy: Cholecystectomy is the surgical removal of the gallbladder. Laparoscopic cholecystectomy involves several small incisions in the abdomen to allow the insertion of the surgical instruments and a small video camera.


Inguinal Hernia Repair

Inguinal Hernia Repair:Hernia repair involves surgery to correct an abnormal bulging of internal organs, often the intestine, through a weakness in a muscular wall. Inguinal Hernia Repair can be performed conventionally with a single long incision or laparoscopically, using several small incisions to allow the insertion of the surgical instruments and a small video camera.

Obesity Surgery


Lap Band
Adjustable gastric banding is a form of restrictive weight loss surgery bariatrics designed for obesity patients with a body mass index (BMI) of 40 or greater - or between 35 – 40 with those who have co-morbidities that are known to improve with weight loss. The gastric band is an inflatable silicone prosthetic device which is placed around the top portion of the stomach via keyhole laparoscopic surgery.
The placement of the band creates a small pouch at the top of the stomach which holds approximately 50 ml. This pouch 'fills' with food quickly and the passage of food from the top to the bottom of the stomach is slowed. As the upper part of the stomach believes it is ‘full’, the message to the brain is that the stomach is full and this sensation helps the person to eat smaller portions, eat less and therefore lose weight over time.

Ophthalmology



Cataract Surgery

A cataract is a clouding of the eye's naturally clear lens. This clouding results in impaired vision. In cataract surgery the surgeon removes the clouded lens and replaces it with a clear plastic intraocular lens.

Lasik

LASIK is a surgical procedure intended to reduce a person's dependency on glasses or contact lenses. LASIK stands for Laser-Assisted In Situ Keratomileusis and is a procedure that permanently changes the shape of the cornea, the clear covering of the front of the eye.


Panretinal photocoagulation

Panretinal Photocoagulation PRP, is a type of laser surgery used to treat severe macular edema and proliferative retinopathy. In treating advanced diabetic retinopathy, the burns are used to destroy the abnormal blood vessels that form at the back of the eye.

Orthopedic



Meniscectomy

Meniscectomy is the surgical removal of all or part of a torn meniscus. A meniscus tear is a common knee joint injury. Surgeons who perform meniscectomies (orthopedic surgeons) will make surgical decisions based on the meniscus's ability to heal, as well as your age, health status, and activity demands.

The choice of type of surgery is based on the extent and location of the tear, your age and activity level, the surgeon's experience, and your preferences. Orthopedists most often perform meniscus surgery with arthroscopy, a procedure used to both examine and repair the inside of a joint. A thin tube (arthroscope) containing a camera and light is inserted through small incisions near the joint; surgical instruments are inserted through other small incisions. Arthroscopic surgery has the potential to limit knee damage due to surgery and to promote fuller recovery. 2 However, some tears may require open knee surgery.

ACL Repair

Anterior cruciate ligament reconstruction (ACL reconstruction) is surgical graft replacement of a torn anterior cruciate ligament in the knee. Because the ACL does not heal on its own, an ACL reconstruction requires a tissue graft. The torn ligament is removed from the knee before the graft is inserted. The types of surgery differ mainly in the type of graft that is used. In all cases, the surgery is done arthroscopically.

No ideal graft for ACL reconstruction exists. All graft choices have advantages and disadvantages. Patella tendon grafts are still considered the historical "gold standard" for knee stability by surgeons, however they suffer a slightly higher complication rate. Hamstring grafts had initial problems with fixation slippage. Modern fixation methods of hamstrings avoid graft slippage, producing outcomes that are the same in terms of knee stability with easier rehabilitation, less anterior knee pain and less joint stiffness. The main factors in knee stability are correct graft placement by the surgeon and treatment of other menisco-ligament injuries in the knee, rather than choice of graft.

Carpel Tunnel Release Surgery

Carpel Tunnel Release Surgery is used to reduce the pressure on the median nerve in the wrist. This is done by cutting the ligament that forms the top of the carpal tunnel. Cutting this ligament relieves pressure on the median nerve.


Hallux Valgus

A bunion (hallux valgus) is an enlargement of bone or tissue around the joint at the base of the big toe (metatarsophalangeal joint). If you have a bunion, you will notice a bump on your big toe joint. The big toe may turn in toward the second toe (displacement), and the tissues surrounding the joint may be swollen and tender.

Surgery to correct a bunion may be considered if other treatment does not relieve pain. There are many different types of surgery for bunions. The type of surgery used depends on the specific type of problem found in the toe bones and soft tissue.


Hip Replacement

In a total hip replacement the diseased cartilage and bone of the hip joint is surgically replaced with artificial materials. Chronic pain and impairment of daily function of patients with severe hip arthritis are reasons for considering treatment with total hip replacement. Physical therapy is an essential part of rehabilitation after a total hip replacement.


Knee Replacement

In a total knee replacement the diseased cartilage and bone of the knee joint is surgically replaced with artificial materials. Chronic pain and impairment of daily function of patients with severe knee arthritis are reasons for considering treatment with total knee replacement. Physical therapy is an essential part of rehabilitation after a total knee replacement.


Rotator Cuff Repair

The rotator cuff is a group of muscles and tendons that form a cuff over the shoulder. These muscles and tendons hold the arm in its "ball and socket" joint and help the shoulder to rotate. The role of the tendons is to hold the powerful shoulder muscles to the shoulder and arm bones. The tendons can be torn from overuse or injury. Rotator cuff repair is a type of surgery to fix a torn tendon in the shoulder. The procedure can be performed with a traditionally large ("open") incision or with shoulder arthroscopy.


Valgus Knee Repair (Osteotomy)

In patients with degenerative arthritis, or osteoarthritis, of the knee joint, deformities of the knee are common. These deformities include a bow-legged or knock-kneed appearance. More technically, these deformities are called genu varum (bow-legged) or genu valgum (knock-kneed).

Knee osteotomy is surgery that removes a part of the bone of the joint of either the bottom of the femur (upper leg bone) or the top of the tibia (lower leg bone) to increase the stability of the knee. Osteotomy redistributes the weight-bearing force on the knee by cutting a wedge of bone away to reposition the knee. The angle of deformity in the knee dictates whether the surgery is to correct a knee that angles inward, known as a varus procedure, or one that angles outward, called a valgus procedure. Varus osteotomy involves the medial (inner) section of the knee at the top of the tibia. Valgus osteotomy involves the lateral (outer) compartment of the knee by shaping the bottom of the femur.
