Ilizarov Rings - Modern limb lengthening

Ilizarov Rings

The Ilizarov Technique allows the orthopedic surgeon to use the patient’s biology to form new bone. When the patient has any one of these:
  • bone loss
  • leg length discrepancy
  • congenital or post-traumatic deformities
  • osteomyelitis
The technique was invented (by an accident) by Gavriel Abramovich Ilizarov from Soviet Republic. Healthbase - Medical Tourism - Gavriil Abramovich Ilizarov - Ilizarov Rings Ilizarov cross-inserted two to three thin, flexible wires (bicycle spokes) through the broken bone above the fracture site, and two to three wires below the fracture site. The wires transected both the fractured bone and the soft tissue. He secured the ends of the upper set of wires to one metal ring encircling the limb and the ends of the lower set of wires to another metal ring (picture two bicycle wheels perpendicular to the limb, one wheel above the fracture, one below). After positioning the bone fragments by manipulating the wires and rings, he anchored the two metal rings to each other via external rods running the length of the limb. Strong, yet flexible, the apparatus accommodated body weight and motion, stimulating the growth of new cells between bone fragments.
Ilizarov developed his ring external fixator in 1951. The first bone lengthening happened quite by accident. He was planning to lengthen an amputation stump in a patient by distracting the ends of the bone after cutting it and then taking bone from the hip to fill the void (bone grafting). The patient was using the fixator to slowly increase the gap between the bone ends as directed. Unfortunately, Ilizarov went on vacation for several weeks and forgot about his patient. When he returned, the patient had lengthened his stump 8 cm (about 3 inches) and there was new bone forming in the gap! He never had to bone graft.
