Dupuytren's contracture

Dupuytren's contracture (also known as Morbus Dupuytren or Dupuytren's disease, and sometimes misspelled as Dupuytren's constricture) is a painless thickening and contracture of tissue underneath the skin on the palm of the hand and fingers. The thickening of the tissue can cause the fingers to curl. Progressive contracture may result in deformity and loss of function of the hand.

Dupuytren's contracture most commonly affects the ring finger and little finger; the middle finger may be affected in advanced cases, but index finger and thumb are nearly always spared.

 

Causes

The cause of this contracture is unknown, but minor trauma and genetic predisposition may play a role. The condition becomes more common after the age of 40, and more often affects men than women.

Certain risk factors associated with Dupuytren's contracture are:

Symptoms

•  Painless nodule in the palm, developing into a cord-like band
•  Thickening of the lines in the palms of the hands
•  Inability to fully extend or straighten the affected finger from the flexed position, which can result in a loss of normal grasping
•  There is seldom any pain associated with Dupuytren contracture unless the affected fingers are forcefully hyperextended

 

Treatment

Non-surgical treatment

Because Dupuytren's contracture usually progresses very slowly, it may not be troublesome for years. Exercises, warm water baths, or splints may be helpful in the initial stages while the progression of the contracture is monitored. Radiation therapy inhibits development of contracture specifically in early stages. If a painful lump is present, a Triamcinolone (kenalog) injection may provide some relief.

Surgical Treatment

Surgery is recommended in cases of severe contracture when the inability to straighten the fingers significantly limits hand function. Surgery for Dupuytren's contracture divides or removes the thickened bands to help restore finger motion. Sometimes the wound is left open and allowed to heal gradually. Skin grafting may sometimes be needed.
