What is a revision knee surgery or revision total knee replacement surgery?

Healthbase - Knee implant components in a total knee replacement TKR

Total knee replacement implants like any other joint replacement implants do not last forever and may fail after 10 to 15 years or sooner. The parts may come loose or they may wear out. A revision knee surgery is, therefore, an operation to replace (in part or in full) a previous knee replacement joint that wears out, loosens or develops a problem. This operation varies from very minor adjustments to massive operations replacing significant amounts of bone.

Why does a knee surgery need to be revised?

Following are the main reasons:
•  Plastic wear: In this revision only the plastic insert is changed.
•  Instability: The knee is not stable and may be giving way or not feel safe when you walk.
•  Loosening: Loosening of either the femoral, tibial or patella component which can be detected on regular follow-up X-Ray examinations.
•  Infection: Infection usually presents as pain but may present as swelling or an acute fever.
•  Osteolysis (bone loss): This can occur due to worn out plastic particles being released into the knee joint which result in bone being destroyed due to an attack by the body's immune system.
•  Ongoing unrelieved pain
•  Recurrent dislocation
•  Fracture
•  Significant leg length discrepancy

How do I know when I need a revision knee replacement surgery?

•  X-Ray exam: Your orthopedic surgeon can detect the need for a revision knee replacement surgery during follow-up X-Ray exams which are recommended to be had lifelong following a TKR operation.
•  Unrelieved pain: Also, if you have unrelieved pain in the replaced knee, your orthopedic surgeon can detect and determine whether you need a knee revision procedure using regular X-ray examinations. If a problem is detected, i.e. if the X-rays of the femur and tibia show damage has occurred to the knee replacement causing its failure, you may need to have a revision surgery before a major problem develops.
•  Routine blood tests especially to rule out infection.
•  Bone scans can help to determine if a component is loose.
•  Aspiration of the joint is occasionally done to diagnose or rule out infection.
