JAW REDUCTION OR MANDIBULAR REDUCTION

Jaw Reduction or Mandibular Reduction is a type of aesthetic plastic surgery to narrow the lower one-third of the face. A slender jaw profile is achieved by reducing the mandible and its muscular attachments.

A wide lower face can primarily be caused by enlargement of the mandible (inferior maxillary bone) or masseter muscle (muscle that assists in chewing). The cause of an enlarged mandible could be either developmental or congenital. There are some rare disorders that can further widen the jaw such as acromegaly. A masseter muscle can be large due to congenital reasons or it can be an acquired deformity. Repeated gum chewing, teeth clenching, or bruxism can contribute to enlargement of the muscle.

The treatment technique is selected by examining the patient to determine the cause of the enlarged jaw which can be due to the bone, the masseter or both. Treatment can be done either non-surgically or surgically.

Non-surgical techniques are essentially limited to cases in which the masseter is enlarged. Non-surgical methods involve botox injections into the enlarged muscle, weakening it so it slowly becomes smaller through atrophy over several months. Improvement is generally not seen for at least 1-3 months. Peak improvement occurs at months 3 to 9 months with good results observable at one year in many patients. A rare complication to this approach is temporary paralysis of the muscles that move the lips.

Surgery is used to directly reduce the size of an enlarged mandible. Incision can be to the inside or outside of the mouth, though internal incision is most common because it leaves no visible scar. A burr is used to remove the outer layer of the enlarged mandible, narrowing the jaw. Potential complications include injury to the inferior alveolar nerve which provides sensation to the lower lips and teeth.
