Dr. Bose's patient: Cory Foulk - endurance triathlete

Dr. Bose

Cory Foulk is an endurance triathlete who competes in races where the bike portion is always more than 100 miles and the run portion is at least the length of a marathon. By mid-2005 his hip pain was so severe that he was going to say goodbye to his 30-year competitive lifestyle. Then he got relief with the BHR Hip.

Towards the middle of 2005, it was the endgame for me. It was awful. That year I decided to say goodbye to triathlon, which I had been competing at marathon distance or greater for almost 30 years at that point. For me that was a significant change of lifestyle.

I knew that if I had a BHR done and it didnt work, then I could have a total hip replacement. I had a fallback position. It was almost because I wasnt willing to commit. I have other options in the future. It was a very conservative approach to that problem.

Birmingham Mid Head Resection Prosthesis (BMHR)

ARCH (Asian Regional Centre for Hip Resurfacing), where Dr. Bose performs Birmingham Hip Resurfacing (BHR) has been selected to be one of the very few centres worldwide to do BMHR ( Birmingham Mid Head Resection prosthesis). This will be the only centre in Asia to offer the Birmingham Mid Head Resection prosthesis.

Birmingham Mid Head Resection prosthesis is an advancement in the BHR and will expand the indications for hip resurfacing surgery. Birmingham Mid Head Resection prosthesis (BMHR) helps advanced aged patients to have Birmingham Hip Resurfacing.

The BMHR has specifically been designed for patients who have weaker bone in the head of the femur.
In the BMHR more of the weak bone is resected and is replaced with metal.
This will dramatically increase the range of patients who can have this surgery.
X-ray wise it looks almost like the BHR but the amount of bone removed is more. Thus the risk of fracture in post-op is reduced or eliminated for borderline patients.
It is technically complex to do and hence has been released only to select surgeons like Dr.Bose around the world.

Video about Advanced spine surgery by Dr. Rajakumar

Larry from Florida, USA talks about his excellent experience after a complex spine surgery at Wockhardt Hospital,Bangalore. Amongst the few hospitals in Asia equipped with most advanced Neuro-navigation and Endoscopic facilities to perform complex brain and spine neurosurgeries and a vastly experienced team of Neurosurgeons and Neurologists, backed by the most comprehensive neuro-diagnostic and imaging facilities, Wockhardt Hospitals in Bangalore India are now the first choice for non insured self paying US patients. Few hospitals even in US can match the neurological services at this Indian hospital that has latest in diagnostic equipment such as EEG, ENMG, and Video-monitoring, 1.5 Tesla MRI and 64-Slice CT Scan.
Our dedicated state-of-the-art Neurosurgery operation theaters have semi-robotic microscopes, the neuro-navigation facility, cranial and spinal endoscopy, MRI and CT compatible stereotaxy for functional neurosurgery that assist surgeons in attaining precision while conducting complex neurosurgeries

Some comments on Dr Hegde spine work

  • Have pioneered the use of many modern instrumentation systems, for Example, the Cotrel Dubousset, the Moss Miami, The Ray cages (PLIF), the BAK cages and the Harms Mesh Systems in India
  • Present repertoire of work covers almost the entire gamut of spinal disorders and includes deformity, low back, tumor, trauma, Infection & cervical spine surgery
  • In deformity surgery, Dr Hegde has successfully managed a wide range of deformities from mild to the very severe using back, front back, multiple staged interventions. Dr Hegde quite at home in the use of anterior as well as posterior instrumentation systems
  • In the low back, Dr Hegde routinely tackle a variety of problems ranging from disk prolapse, to spondylolisthesis, failed back and salvage situations. I do complicated revision surgeries for instability and sagittal malalignment using back-front-back techniques in the same sitting. I am familiar with PLIF (BAK cages), TLIF (Harms technique) and global fusion techniques
  • Dr Hegde have tackled variety of tumors including benign (eg. Hemangioma) and malignant (primary, secondary). Dr Hegde familiar with vertebrectomy techniques and reconstruction using cages, grafts, etc.
  • Dr Hegde have tackled trauma using a wide variety of approaches concepts and implant systems, from the front and back
  • Dr Hegde frequently invited nationally as well as internationally to conduct workshops on modern spinal techniques and perform live demonstration surgeries.
  • Teaching responsibility
    • Guide in the DNB teaching program
    • Spine fellowships training of aspiring spine surgeons from India and abroad
    • Dr. Hegde credited with introducing Arthroplastic reconstruction of the lumbar and cervical discs with artificial discs in India.

A first for an Indian spine surgeon:

Dr Hegde designed in collaboration with Medtronic Sofamor Danek (MSD) a comprehensive spinal instrumentation system for anterior fixation in a variety of spinal problems including spinal deformities, tumors, infections and injuries. The system is awaiting FDA clearance and will be marketed internationally by MSD. It is called The Pegasus .

SPECIALITY INTEREST
* Primary and Revision joint replacements Hip, Knee & Shoulder
* Complex joint replacements in dysplastic joints, stiff joints
* Minimally invasive hip & knee replacements
* Oseotomies, pelvic & acetabular trauma
* Salvage & Limb reconstruction in skeletal tumors
* Pediatric problems of the hip

Current fields of interest are evaluating strategies for minimal invasive surgeries, joint replacements and revision replacements of hip, knee and shoulder joints. We see more and more of technically demanding reconstructive challenges. Reconstruction following tumour resection has been his other field of interest. Over the next few years we intend to expand this into one of the advanced facilities in this area.
